Your right to a Good Faith Estimate
Under federal law (the No Surprises Act, effective January 1, 2022), healthcare providers must give patients who are uninsured or self-pay a written estimate of the expected charges for medical services, including psychotherapy services. You have the right to receive this estimate before you receive care.
What this means at 360° Therapy
If you are paying for therapy without using insurance — either because you don't have insurance, or because you've chosen not to use it for this care — you are entitled to a written Good Faith Estimate of what your therapy will cost.
Your estimate will include:
- The expected charge per session at your assigned clinician's tier ($175, $210, or $300)
- An estimated number of sessions over the next 12 months, based on your initial clinical picture
- The cumulative estimated cost over the estimate period
- Anticipated billing codes (most commonly CPT 90837 for 60-minute individual psychotherapy, or 90834 for 45-minute sessions)
- Your clinician's name, license, and National Provider Identifier (NPI)
- The address of our practice
You will receive your written Good Faith Estimate:
- Within 1 business day of scheduling, if your first session is scheduled at least 3 business days out
- Within 3 business days of scheduling, if your first session is scheduled 10 or more business days out
- Upon request, at any time — even before you book
Important things to know about your estimate
It's an estimate, not a contract. The actual number of sessions, cadence, and total cost depend on the work you and your clinician do together. Some clients need fewer sessions; others need more. The estimate is our best forecast based on the initial picture.
You can ask for an updated estimate at any time. If your treatment plan changes meaningfully — for example, you shift from weekly to biweekly sessions, or you add a specialty service like EMDR — you can request an updated estimate.
The estimate covers care from 360° Therapy only. Any other providers you choose to see (psychiatrist, primary care physician, other specialists) will provide their own estimates.
If your final bill exceeds the estimate by $400 or more
If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill through the federal Patient-Provider Dispute Resolution process. You must initiate the dispute within 120 calendar days of receiving the bill.
To start a dispute, visit www.cms.gov/nosurprises or call 1-800-985-3059. There is a $25 administrative fee to initiate the dispute, which is refundable if you prevail.

Insurance clients
If you are using in-network insurance, the No Surprises Act Good Faith Estimate requirement does not apply to your sessions in the same way — your insurance contract governs the cost-sharing. We still verify your benefits before your first session and tell you exactly what your copay or coinsurance will be.
If we are not in-network with your insurer but you plan to submit for out-of-network reimbursement, we will provide both a Good Faith Estimate and a superbill compatible with your insurer's reimbursement process.
